HRT Mental Health Forms

I know, for myself at least, one of the hardest things about being a trans man is dealing with dysphoria about my body. I see other boys my age who are sprouting facial hair, and their voices are cracking, and I look like I'm ten years old because I'm on hormone blockers, so I've never been through any sort of puberty. I am in the process of beginning testosterone treatments, and one thing that I had to do was bring in a letter from a gender identity specialist saying that I am mentally stable and that I understand the risks and whatnot, and a letter from a regular therapist saying that I have no preexisting conditions that are causing me to make bad decisions about beginning testosterone. It was confusing to do because I wasn't sure, exactly, what they were supposed to write. I've written out sample letters for both a gender specialist and mental health specialist. You can use these simply as a format for your therapists to use, or you can simply fill in the blanks.
